Only BPL category are eligible - As a proof ration card to be attached.
Financial support will be considered based on age of applicant and Illness.
Only bills above Rs 25000 for a treatment are eligible. Bills dated within 3 months of treatment completion only will be considered.
Those covered by ESIC, medical insurance, free treatment at government hospital and other medical support from elsewhere such as Chief Minister’s medical assistance, Prime Minister’s medical assistance, and crowdfunding, are not eligible.
A recommendation letter addressed to K Chittilappilly Foundation issued within 3 months of the date of application from a Ward Member/Councilor/MP/MLA should be attached.
